History: Wiki links are fine! If you are apping an OC or if Wiki links are insufficient, please write a summary (2-4 paragraphs) of their history.
Personality: We will accept bullet points of your character's strengths and weaknesses (at least 3 each), provided that you elaborate on each. We don't have a sentence limit, just give us canon examples! If writing a standard personality section works better for you, we'll accept that as well.

Abilities: Powers are nerfed here, but as characters may have an opportunity to regain them momentarily, we ask that you fill out this section anyway. Any other skills can be listed here as well!

Item Regains: List up to five items that are from your character's home world. They can be significant to your character or another character from your canon. For example, Joker from Persona 5 can regain Ryuji’s Skull mask and/or his own mask. No drugs, alcohol, weapons and animal companions. Nothing over six feet, either (i.e. Voltron Lions). Items with magical properties must be de-powered. Toy or plush versions of these prohibited items except for drugs and alcohol, however, are acceptable.

Please format each regain as Item Name: Brief description of item. One or two sentences per item will suffice. As an example: "A bag of cookies: Chocolate chip cookies that smell freshly baked. Aren't you lucky!"

Physical Form: Here is where you can specify if you'd like to A) keep your character 100% human/their original form, B) keep your character in their original form with animal traits (i.e. cat ears and tail), or C) go full animal. Should you choose B or C, (as of 5/15) you may choose any animal species you'd like, in-game or not. We will allow animals with the appearance of a mythical creature (i.e. horse as unicorn for Julian, alligator as a dragon for Drago).

Title: This part isn't game-changing, it's just a fun little feature we wanted to include! In Animal Crossing: New Horizons, you're able to create a title from a list of keywords (scroll down to "Passport Titles"). You're free to create a title for your character that may or may not come up in-game, or have us create one for you! The choice is yours! ICly, characters will be given these titles upon arrival.

Extra notes: In case there’s anything else you’d like to mention about either yourself or your character! Opting out of the wasp/scorpion instances can go here too.
